Potchefstroom - A tired driver was likely to have been the cause of a head-on collision on the road between Ventersdorp and Klerksdorp in which 12 people died about 05:00 on Tuesday morning.
Senior Superintendent Pieter van Zyl said according to the truck driver, a Toyota Venture travelling in the opposite direction swerved into his path.
The truck driver tried to avoid the Venture twice, but the Venture swerved again.
Four men, four women, three girls and a boy travelling in the Venture died on impact.
The truck driver and his assistant were seriously. They are being treated at the Ventersdorp hospital.
The Venture, from Umtata in the Eastern Cape, was travelling to Rustenburg.
