Why would i have serious lower back pain as well as pain in my hands and feet with a stomach bug. fever as well

You may have a bacterial infection with referred pain to the back. I am not sure what the cause of the pain in your hands and feet are as this is not normally related to diarrhoea. You probably need antibiotics, so please see your doctor for an examination, to send a sample of stool to the lab and for a prescription.
