I KNOW ..i know
how does a therapist go to work to treat some1 with low self esteem(confidence aswell - the 2 go hand in hand), and a bad self c i really need 2 change and quick cause im going down a self destructive path and the more i think about it the worse it gets...i know why ive got the long would it take 2 rectify...ive been feeling this way for a year or so...lost most of the friends i care about due to this

Well, you've already made the most important and essential first step, on your own --- to recognize that there is a problem and that you need and deserve help with it. In my view, the most effective method now available would be CBT (Cognitive-Behaviour Therapy ) --- a style of counselling with good evidence from research, that it is effective and that it doesn't take long to produce lasting and useful results. As its name implies, it focusses on Cognition ( the way you think about yourself and the world ) and Behaviour ; it helps to analyze the ways in which things are not working right, the havits of thought and the often unrealistic expectations you have formed ; and it not only is well geared to help with the sort of problems you describe, but should teach you HOW to deal productively with similar problems should they arise in the future. And unlike some other methods, it doesn't take long to show its effectiveness.
And congratulations on your success in stopping the smoking --- especially in resisting the temptations to slide back when among smoking friends. Making some non-smoking friends would help, too
