Dear Smee

Insulin perse does not have any significant bearing on fertility. However, in the presence of polycystic ovarian disease, insulin levels have been shown to be unusually elevated(insulin resistance). This is because of the close genetic link in terms of position of genes or chromosomes between the polycystic ovarian disease and diabetes. In the combined situation, the hyperinsulinanemia seems to interfere with ovulation aswell as ferilisation capacity and also may have impact on implantation and early development of the embryo.
Patients who have been treated with weight reduction, diet and insulin lowereing agents such as glucophage, have been shown to ovulate better and conception rates in such patients have definetly improved as a result.
