What fertility treatment route do we go?

We have gone through 2 unsuccessful IUI treatments (with no medications, only Pregnyl trigger injection). I will go for my 3rd tomorrow and the next day.

I am unsure what to do if this one doesn't work. My husband has a good sperm count (40 mil/ml squared) but only 10% motility.

I have very irregular periods and am not sure that I ovulate every month. My ovaries are polycystic but I don't have the syndrome. My HSG came out fine.

I am 24 years old and my husband is 26. Where to from here? Do we try IUI with meds or move on to IVF?

I would suggest that you attempt another IUI cycle but in the next cycle ensure that it is by means of a stronger stimulation protocol. This should be done under the supervision of a fertility specialist and I would recommend that a combination of clomiphene citrate (clomid, fertimed, serofene) together with Hmg injections (human menopausal veneletropa or equivalent) should be done.
I recommend that monitoring be done with ultra sound scanning and that ovulation be induced at the appropriate time. Should the follicle numbers between the two ovaries exceed 5, it would be feasible at that point to convert to an IVF cycle without excessive costs being involved. At our centre the additional costs would be in the region of between R1 000 and R 2000 at the most.
