A 34-year-old mental healthcare patient at the Thabamoopo Psychiatric Hospital died after allegedly trying to escape from a ward early on Sunday.
Limpopo health department spokesperson Neil Shikwambana claimed the patient removed part of a window frame from the facility in Lebowakgomo.
Shikwambana said the outside burglar-proof window trapped the patient.

"The nursing staff, after hearing the noise at the window, rushed to see what is happening. They discovered the patient trapped and hanging from the window.
"The South African Police Services [SAPS] and forensic pathology were immediately called to the scene. The SAPS opened an inquest case," he added.
Health MEC Dr Phophi Ramathuba has sent her condolences to the patient's family.
