The Crusaders, who won the last of their seven titles back in 2008, finished second in the overall standings, beat the Highlanders 17-0 in the quarter-finals and the Chiefs 27-13 in the semi-finals.
The Lions, who finished the regular season stage of the tournament atop the log, edged past the Sharks 23-21 in their quarter-final clash, before coming back from a 22-3 deficit to beat the defending champion Hurricanes 44-29 in the semi-finals.
Victory for the Lions would see them become only the second South African side (after the Bulls) to land the southern hemisphere's most competitive tournament since inception in 1996.
Kick-off at Ellis Park is at 16:00.
